Patient check-in info and form links in one place

To use a QR code for patient check-in, link it to a clinic info page that points patients to your own forms and portal. CirculateQR's Healthcare template shows opening hours, a what-to-bring checklist, links to new-patient forms, the patient portal and billing, your address with directions and the front desk number. Don't collect medical details through CirculateQR; keep those in your patient portal. Update hours and notices without reprinting signs.

The problem

Why the printed version falls short

Patients arrive without their ID, insurance card or medicine list

New patients can't find the right intake form

Front desk staff repeat directions and parking details all day

Printed notices about hours or closures go out of date

The patient portal's web address is long and hard to type

Features

Useful extras

Checklist

Patients tick off items like photo ID and insurance card on their phone. Ticks stay on the phone; nothing is sent.

Copy buttons

Mark details like the guest WiFi password as copyable.

Anonymous scan counts

See how often the page is opened, by day and time, without names. IP addresses are stored only as a salted hash.

A plan badge means that plan or higher is needed. Paid marketplace templates show their one-time price. Compare plans

FAQ

Frequently asked questions

Is this HIPAA compliant?

No. CirculateQR isn't built for protected health information, so don't collect medical details through its pages or forms. Use it for check-in information, directions and links, and keep anything medical in your own patient portal.

Can it connect to our EHR?

No. There's no EHR integration. The page links to your existing patient portal and forms, and what patients enter there stays in that system.

Can patients fill in intake forms on the page?

Link to the intake forms in your own patient portal. CirculateQR has form pages, but they aren't meant for medical details.

Can we see which patients scanned?

No. Scans are anonymous: you see counts by time, device and approximate city. IP addresses are stored only as a salted hash.

What happens to the code if we stop paying?

If you cancel a paid plan, codes keep working while the subscription is active and for 14 days after it ends. Then they pause until you subscribe again. Nothing is deleted.
