NPS Survey: NPS survey that routes customers by score
NPS Survey asks the 0–10 “How likely are you to recommend us?” question and changes the follow-up by score: high scorers are invited to review you on the site you choose, low scorers get private questions and trigger an alert email to you, and Responses shows your NPS over time.

What’s included
Score-based routing
You set the thresholds (9–10 high and 0–6 low to start). Follow-up questions change with the score, and high scorers see a review button for Google, Yelp or another site you link. Everyone else sees a quieter review link too, because Google doesn’t allow asking only happy customers.
Low-score alerts
Low scorers can tick “You can contact me”, and you get a “Low score” email even when other response emails are off.
NPS summary
Responses shows your Net Promoter Score, the promoter, passive and detractor split, and a weekly or monthly trend chart.
Who it’s for
- Cafés, restaurants and shops asking for feedback
- Clinics, gyms and service businesses
- Teams tracking Net Promoter Score
How to use it
- 1
Buy NPS Survey in the marketplace for $3.99, paid once. Then open it in Studio, filled with the sample content shown in the preview.
- 2
Replace the sample content with yours. You can change: score question and scale labels; thresholds; review site and link; follow-up questions; contact and alert switches.
- 3
Choose how the QR code looks: a plain code, your brand colours, or AI art.
- 4
Publish. CirculateQR hosts the page and gives you the QR code to download as PNG, SVG or PDF.
- 5
Share the code, then read answers on the code’s Responses page or download them as a CSV.
Good to know
- What it collects: Scores and answers go to the code’s Responses page, with the NPS summary.
- Routing uses the 0–10 score, not the words people write.
- Add your review link before publishing; the sample has none.

Frequently asked questions
How much does NPS Survey cost?
NPS Survey costs $3.99, paid once in the CirculateQR marketplace. There is no subscription for the template. It also comes in the Digital Agency Bundle ($24.99).
Do I need a paid plan to use it?
No. You can publish on the free Standard plan, which includes up to 3 hosted pages. On the free plan a code and its page stay live for 14 days after publishing; after that, $5 once keeps one code live for good, or any paid plan (from $12 a month) keeps all your codes live.
Can I change the page after the QR code is printed?
Yes. Edit it in Studio and publish again. The printed code keeps pointing to the same page, so nothing needs reprinting.
Do visitors need an app?
No. The page opens in the phone’s web browser straight from the camera scan.
Where do the answers go?
Every response is saved to the code’s Responses page, where you can download a CSV and choose whether to get an email for each one.
