Pre-shift safety checks at the equipment, signed off by phone
To run safety checklists with QR codes, open the Safety Checklists template in Studio and make a page for each machine or area: the checks to work through, what to do if something fails, a supervisor contact and a sign-off form. Post the code at the spot. Workers tick the checks on their phone, then send the sign-off with their name, the result and any notes. Sign-offs appear in the code's Responses with the time received and download as CSV.

Start from the template
Open Safety Checklists in Studio and replace the sample forklift check with your machine or area.Write the checks and results
List each check, then set the sign-off answers, such as All checks passed or Problem found, tagged out.Post the code
Print one code per machine or area and fix it where the check happens, somewhere with WiFi or mobile signal.Review sign-offs
Open the code's Responses to see who signed off, the result, notes and time, and download a CSV when you need records.

Frequently asked questions
Are the ticked boxes saved?
No. Ticks stay on the worker's phone. What's saved is the sign-off form: their name, the result they pick, any notes and the time it arrived.
Does it alert a supervisor when a check fails?
No alerts are sent. Supervisors review sign-offs in Responses. Put the supervisor's number on the page so workers can call straight away.
Can it prove who did the check?
No. The worker types their name, and there's no photo upload, identity check or location check. Treat sign-offs as a log, not proof.
Does this meet OSHA or other recordkeeping rules?
CirculateQR isn't certified for any standard and can't guarantee an audit outcome. Check your recordkeeping requirements with your safety advisor.
What if there's no signal?
The page needs WiFi or mobile data to load and to send the sign-off. Keep a paper backup for dead zones.
What happens on the free plan?
The code pauses 14 days after publishing until you upgrade. On a paid plan it keeps working while you subscribe and for 14 days after you cancel.
